Have a schedule change?

If you are planning to cancel more than 2 consecutive days of treatment, we ask that you please provide advanced notice by completing our Schedule Change Notification Form in addition to notifying your child’s treatment team. Providing advanced notice will give us the opportunity to create a support plan for while you are away and will help ensure minimal disruption to your child’s treatment team.
